Roles & Responsibilities

• Perform thorough evaluations and develop individualized treatment plans focused on patient function and goals
• Deliver skilled therapeutic interventions including mobility training, therapeutic exercise, and functional restoration
• Document patient progress accurately and timely in the clinical record to support outcomes and continuity of care
• Collaborate with interdisciplinary team members, families, and caregivers to coordinate safe discharge plans
• Educate patients and families on home programs, adaptive strategies, and injury prevention
• Ensure equipment is maintained and used safely, and adhere to facility policies and regulatory standards
• Contribute to quality improvement initiatives and participate in team meetings to enhance patient outcomes

Qualifications

• Current California Physical Therapist license or eligible to obtain licensure
• Licensed and in good standing with state regulatory board
• Basic Life Support (BLS) certification or willingness to obtain
• Experience in skilled nursing, inpatient rehabilitation, or outpatient orthopedics preferred
• Strong clinical reasoning, patient-centered communication, and documentation skills
• Ability to work collaboratively in a multidisciplinary care environment

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in Physical Therapy or related field considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual therapy techniques
• Knowledge of mobility training, transfers, gait training, and assistive device progression
• Familiarity with documentation standards, regulatory requirements, and outcome measures
• Excellent interpersonal skills for patient and family education and team collaboration
• Ability to prioritize caseloads and adapt treatment plans to changing patient needs
• Comfortable working with medically complex and post-acute rehabilitation populations
