Roles & Responsibilities

• Administer medications to residents following the 5 Rights and facility policy
• Assess residents and report any changes in condition to the charge nurse and physicians
• Communicate updates to family members and document interactions clearly
• Assist CNAs with resident care tasks, including answering call lights and other hands-on duties
• Report any allegations of abuse, neglect, or misappropriation to leadership promptly
• Perform other resident-centered duties as requested to support quality care

Qualifications

• Current Wisconsin LPN license required and must be maintained
• Current CPR/BLS certification required
• 1+ years of related long-term care experience preferred
• Exceptional accuracy and strong attention to detail
• Able to communicate clearly with physicians, families, and the care team
• Comfortable assisting CNAs and collaborating in a long-term care environment

Education

• LPN diploma or equivalent (or an equivalent combination of education, training, and experience)

Knowledge & Skills

• Medication administration and medication safety practices
• Patient/resident assessment and recognizing change in condition
• Accurate reporting and documentation practices
• Experience in resident care within long-term care settings
• Strong communication skills with families, physicians, and nursing staff
• Teamwork and ability to assist CNAs with daily resident needs
