Roles & Responsibilities

• Collaborate with RN Case Manager to coordinate patient care and smooth transitions
• Document assessments, care plans, and discharge information clearly and thoroughly
• Provide patient- and family-centered support to build trust and facilitate care decisions
• Prioritize and shift attention between tasks to meet high-volume unit needs
• Coordinate post-acute care resources and communicate effectively with interdisciplinary teams
• Participate in self-scheduling and float where needed to support unit staffing

Qualifications

• Minimum 2 years of experience in social work or related healthcare setting (per posting)
• Strong documentation and communication skills as required by the unit
• Comfort collaborating closely with RN Case Managers and interdisciplinary teams
• Certifications: None required (posting indicates NA)
• Familiarity with post-acute documentation expectations and workflows

Knowledge & Skills

• Proficiency with documentation systems: CERNER, NATE, ENSOCARE (post-acute systems)
• Excellent written and verbal communication
• Ability to multitask and reprioritize in a busy hospital environment
• Skill at building rapport and trust with patients, families, and staff
• Team collaboration and care coordination experience
