Roles & Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical challenges and develop individualized, regulation-compliant plans of care
• Use standardized assessments and evidence-based practice to drive intervention decisions
• Deliver skilled rehabilitation interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and assistive device training
• Document treatment plans, progress, and minutes accurately in the electronic medical record per regulatory and company standards
• Assess and adjust treatment based on outcomes, making referrals when appropriate to optimize patient progress
• Provide patient, family, and caregiver education and counseling to support functional independence
• Participate in patient care conferences, utilization review, and rehabilitation meetings to coordinate care
• Supervise and mentor occupational therapy assistants, rehabilitation aides, and students when applicable

Qualifications

• Active Occupational Therapy license for Missouri (compact allowed) required at time of submission
• Minimum 1 year of OT experience required; LTC or rehab experience required
• NBCOT and AOTA-aligned practice expectations; adherence to state practice acts and professional ethics
• New graduates considered if they have Level II experience (not limited to Level I observation)
• Comfortable with electronic documentation/EMR and accurate regulatory documentation
• Ability to obtain necessary DME and supplies and to participate in facility-required trainings

Knowledge & Skills

• OT evaluation and treatment planning with emphasis on ADL assessment and retraining
• Skilled rehabilitation interventions: therapeutic activities, therapeutic exercises, and modalities
• Experience with standardized assessments to measure progress and outcomes
• Proficiency with documentation and electronic medical records
• Knowledge of assistive devices, DME, and safe device implementation
• Ability to supervise OT assistants and collaborate with interdisciplinary teams
• Strong communication and patient/caregiver education skills
