Roles & Responsibilities

• Provide compassionate, patient-centered care that promotes comfort and dignity
• Administer medications and treatments accurately and safely according to policy
• Perform thorough patient assessments and communicate changes promptly to the care team
• Document care clearly and timely in the facility records and electronic charting system
• Assist with activities of daily living and support patient mobility and independence
• Collaborate with nurses, therapists, and interdisciplinary staff to deliver coordinated care
• Maintain a safe, clean environment and adhere to infection control and safety protocols

Qualifications

• Current, active LPN license in Pennsylvania or eligibility to obtain licensure
• Basic Life Support (BLS) certification required or willing to obtain
• Reliable clinical skills in medication administration, patient assessment, and ADL support
• Ability to pass background check, drug screen, and required health screenings
• Strong communication skills and a team-oriented attitude

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Solid knowledge of practical nursing fundamentals and long term care practices
• Medication administration and documentation best practices
• Patient assessment and vital signs monitoring
• Familiarity with electronic health records and accurate charting
• Infection control standards and safety protocols
• Empathy, critical thinking, and clear communication with patients and families
