Roles & Responsibilities

Conduct comprehensive psychosocial assessments to identify patient needs and strengths
Develop individualized treatment plans grounded in evidence-based interventions
Deliver therapeutic interventions across the lifespan, addressing trauma, depression, anxiety, substance use, and adjustment disorders
Collaborate with primary care providers, nurses, and behavioral health professionals to coordinate whole-person care
Provide care coordination and referrals to community resources serving rural and underserved populations
Maintain timely, accurate documentation and contribute to quality improvement efforts
Advocate for patient needs and support family engagement in treatment
Participate in interdisciplinary meetings to ensure integrated, person-centered care

Qualifications

Active LISW license in Iowa (required)
Master's degree in Social Work from an accredited program (required)
Minimum of 2 years of clinical experience preferred
Experience working in rural or medically underserved settings is a plus
Clinical experience with evidence-based therapeutic interventions and trauma-informed care
Strong interpersonal, organizational, and documentation skills

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Psychosocial assessment and treatment planning
Evidence-based therapeutic interventions (individual, family, group)
Trauma-informed care and substance use treatment approaches
Care coordination and referral to community resources
Interdisciplinary collaboration within integrated care teams
Clinical documentation best practices and electronic medical record use
Cultural competence and experience serving underserved populations
Crisis intervention and risk assessment
