Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Perform assessments and monitor residents’ vital signs and clinical status
• Administer medications and treatments safely and accurately according to policy
• Document care, observations and changes in condition clearly and timely
• Collaborate with nursing team and interdisciplinary staff to coordinate resident care
• Support resident activities of daily living and promote a comfortable environment
• Communicate with residents and families with empathy and professionalism
• Participate in required competency testing and maintain compliance with facility protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license — license verification required
• BLS (Basic Life Support) certification required
• Must meet Trinity-specific competency testing requirements
• Current immunizations and health screenings as listed by the facility (PPD/IGRA, MMR, Hep B, Varicella, Tdap, Influenza, COVID-19 as required)
• Fit test and respirator evaluation completion
• Successful completion of Trinity 10-panel drug screen and health statement

Education

• Graduate of an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Solid knowledge of medication administration and basic nursing procedures
• Strong clinical assessment and documentation skills
• Infection control practices and safe use of PPE
• Effective communication and team collaboration
• Ability to follow facility policies, protocols and competency standards
• Comfort working in a per diem/pool environment with variable schedules
