Roles & Responsibilities
- Provide compassionate, safe nursing care to skilled nursing facility residents
- Administer medications and treatments accurately and document thoroughly
- Perform focused assessments and communicate changes in condition to the care team
- Assist with activities of daily living while promoting resident dignity and independence
- Collaborate with nurses, therapists, and other staff to deliver person-centered care
- Support infection control and safety practices in accordance with facility policies
Qualifications
- Valid LPN license in good standing (current or compact where applicable)
- Experience or comfort working in a skilled nursing facility preferred
- Up-to-date immunizations and health documentation as required: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
- Current physical/health statement and ability to pass Trinity 10 panel drug screen
- Strong communication skills and the ability to work well in a team
- Reliable, professional, and patient-focused approach to care
Education
- Graduate of an accredited practical nursing program and active LPN license
Knowledge & Skills
- Medication administration and basic IV/shot experience as applicable
- Resident assessment, documentation, and care planning
- Basic wound care and catheters/ostomy care familiarity
- Infection prevention and control practices
- Time management and prioritization in a fast-paced SNF environment
- Empathy, patience, and strong interpersonal skills
