Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to PACE participants
• Administer medications and treatments safely and accurately
• Perform routine assessments and document clinical findings in the chart
• Collaborate with the interdisciplinary team to support participant health goals
• Assist with activities of daily living and encourage independence
• Follow infection control policies and ensure participant safety
• Support smooth transitions of care and communicate changes to the care team

Qualifications

• Current Licensed Practical Nurse (LPN) license, or eligible for licensure in Louisiana
• Current CPR certification (or obtain upon hire if required)
• Up-to-date immunizations as listed by employer: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
• Clear Trinity 10 panel drug screen and completed physical/health statement
• Comfortable working in a per diem PD Pool model and adapting to varied assignments
• Strong communication skills and a heart for working with older adults

Education

• Graduate of an accredited practical nursing program, or equivalent
• Active LPN licensure required

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic assessment skills and recognizing changes in condition
• Familiarity with infection prevention and standard precautions
• Comfortable with electronic health records and accurate charting
• Team collaboration and clear, professional communication
• Ability to prioritize tasks and manage time during variable shifts
