Roles & Responsibilities

• Provide compassionate direct patient care, focusing on safety and comfort
• Administer medications and treatments according to plan and scope
• Monitor and document patient status, reporting changes to the care team
• Assist with activities of daily living and mobility support
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Support the rehabilitation team in implementing care plans and progress notes
• Maintain accurate, timely documentation and follow facility policies

Qualifications

• Active LPN license in the state of practice (must be verifiable)
• BLS (Basic Life Support) current certification
• Required health screenings and immunizations per facility (TB, MMR, Hepatitis B, Varicella, COVID-19, Tdap)
• Successful background checks and drug screen as part of credentialing
• Prior experience in nursing or rehab settings preferred but new grads with strong clinical skills welcome

Education

• Completion of an accredited Practical Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with documentation in electronic health records
• Excellent communication and patient education abilities
• Team-oriented mindset and ability to collaborate across disciplines
• Adaptability to variable per-diem schedules and changing patient needs
• Basic rehab nursing knowledge and restorative care principles
