Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents and patients in a long-term care setting
• Administer medications and treatments safely and accurately according to facility policy
• Monitor and document patient conditions, vital signs, and responses to treatments
• Collaborate with the interdisciplinary care team to develop and update individualized care plans
• Communicate clearly with patients, families, and staff to ensure continuity of care
• Maintain accurate, timely clinical documentation and adhere to regulatory standards

Qualifications

• Current Licensed Practical Nurse (LPN) credential
• Active authorization to practice per facility requirements; ability to obtain state licensure or endorsement as needed
• Comfortable working 12.5-hour day shifts and managing a full patient assignment
• Strong communication, critical thinking, and teamwork skills
• Reliable, professional, and patient-centered approach to care

Education

• Completion of an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Skilled in basic patient assessment and monitoring
• Competent in medication administration and safe documentation practices
• Knowledge of infection control and safe patient handling
• Ability to prioritize tasks and respond to changing clinical needs
• Good interpersonal skills for working with residents, families, and the care team
