Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents and patients
• Administer medications and treatments safely and accurately per orders
• Monitor and document vital signs, patient status changes, and interventions
• Collaborate with the interdisciplinary care team to coordinate patient care
• Perform thorough and timely documentation to support continuity of care
• Assist with admissions, discharges, and transitions while maintaining patient dignity
• Support infection control practices and maintain a safe care environment
• Participate in shift handoffs and communicate critical information clearly

Qualifications

• Active LPN or RN license (as applicable) in good standing
• Clinical experience in long-term care or acute care settings preferred
• Ability to administer medications and perform routine nursing procedures
• Strong communication and teamwork skills; comfortable in a fast-paced environment
• Demonstrated professionalism, reliability, and patient-focused attitude

Knowledge & Skills

• Patient assessment and monitoring
• Medication administration and documentation
• Accurate charting and electronic health record familiarity
• Basic wound care and infection prevention practices
• Effective communication with patients, families, and care teams
• Time management and prioritization across multiple patients
