Roles & Responsibilities

• Evaluate patients and develop individualized physical therapy plans to restore mobility and independence
• Deliver hands-on and evidence-based interventions to improve function and reduce pain
• Collaborate with rehab team members, nursing, and physicians to coordinate patient-centered care
• Document progress and outcomes clearly and timely to support continuity of care
• Educate patients and families on home programs and safe mobility strategies
• Contribute to a positive therapeutic environment that motivates recovery and celebrates progress

Qualifications

• Current state Physical Therapist license (required)
• Doctor of Physical Therapy (DPT) or equivalent clinical degree
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Experience in inpatient rehabilitation or post-acute settings preferred
• Strong clinical reasoning, documentation skills, and teamwork mindset
• Ability to work Monday–Friday daytime schedule and adapt to facility protocols

Education

• Doctor of Physical Therapy (DPT) or equivalent clinical degree

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and mobility training
• Knowledge of discharge planning and functional outcome measurement
• Familiarity with electronic medical records and timely charting
• Ability to assess fall risk and implement safety strategies
• Excellent communication skills with patients, families, and interdisciplinary teams
• Comfortable treating adults across a range of post-acute and rehab diagnoses
