Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate social work services to patients and families
Coordinate care plans that support patient well-being and recovery
Conduct thorough assessments to identify patient needs and resources
Collaborate with interdisciplinary teams to deliver comprehensive care
Support patients through mental health evaluations and crisis intervention

Qualifications

Active LCSW license in California
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities
Strong communication and interpersonal skills

Knowledge & Skills

Expertise in involuntary psychiatric evaluation processes
Understanding of mental health crisis intervention
Ability to work collaboratively within care coordination teams
Knowledge of community resources and support services
