Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertification, and resumption of care visits for home health patients
Manage a caseload of approximately 6-7 points per day, including regular visits and admissions
Collaborate with sister agencies and clinical coordinators to ensure seamless patient care
Perform safety evaluations and occasional hospice teaching visits as needed
Document patient progress and outcomes accurately and timely

Qualifications

Active North Dakota Physical Therapy License
Current BLS certification
Valid driver's license and auto insurance
Home health experience and OASIS training preferred
First-time travelers welcome to apply
Willingness to work with a preceptor during a 1-2 week orientation

Knowledge & Skills

Home health and OASIS documentation
Start of care and recertification procedures
Strong clinical assessment and problem-solving skills
Effective communication and collaboration with interdisciplinary teams
Time management to handle a daily caseload of 5-6 visits
