Location: Kendallville, IN

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Perform comprehensive OASIS assessments and develop individualized care plans
- Provide skilled physical therapy for homebound patients, focusing on post-surgical recovery, falls, and fractures
- Collaborate with interdisciplinary team to ensure coordinated care
- Educate patients and families on exercises and safety measures
- Document patient progress and adjust treatment plans as needed
Qualifications
- Active Physical Therapist license in Indiana
- Experience in home health or geriatric care preferred
- Ability to complete OASIS documentation accurately
- Valid driver's license and reliable transportation
- Compassionate and patient-centered approach
Knowledge & Skills
- Proficiency in OASIS assessments and start-of-care processes
- Strong clinical reasoning and problem-solving skills
- Excellent time management and documentation abilities
- Ability to work independently in a rural setting
- Knowledge of safety protocols for home environments
