Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide one-on-one physical therapy care to patients in their home environment
Develop and implement personalized treatment plans to improve mobility and function
Collaborate with physicians, families, and interdisciplinary team members
Educate patients and caregivers on exercises and safety strategies
Document patient progress accurately in HomeCare/HomeBase EMR
Cover a 60-mile radius around Valley City, ND, bringing care to those who need it

Qualifications

Active North Dakota Physical Therapy license (or compact eligible)
BLS certification from the American Heart Association
Valid Driver's License and current Auto Insurance
Minimum 1 year of home health experience
Ability to travel up to 60 miles from Valley City

Knowledge & Skills

Proficiency with HomeCare/HomeBase or similar EMR systems
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Comfortable working independently and managing a caseload of 5-7 patients per day
