Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturday once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess, evaluate, and update therapeutic/rehabilitative/functional status of patients
Develop and implement individualized physical therapy treatment plans
Instruct patients and families in the use of therapeutic appliances
Coordinate with physicians and other healthcare professionals to ensure comprehensive care
Maintain accurate clinical and progress summaries
Supervise home health aides when participating in patient rehabilitation
Evaluate home environment and recommend adaptive equipment

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Excellent oral and written communication skills
Strong organizational skills and ability to work independently

Knowledge & Skills

Physical therapy assessment and treatment techniques
Home health and rehabilitation best practices
Patient education and family training
Documentation and compliance with Medicare/Medi-Cal regulations
Interdisciplinary collaboration
