Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination and discharge planning for patients
Conduct thorough psychosocial assessments to support individualized treatment plans
Collaborate with interdisciplinary teams to ensure seamless patient transitions
Facilitate 5150 evaluations and involuntary hold processes when clinically appropriate
Connect patients and families with vital community resources and support services
Document all interventions accurately and maintain compliance with regulatory standards

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities strongly preferred
Strong assessment and crisis intervention skills

Knowledge & Skills

Expertise in involuntary psychiatric holds and 5150 processes
Knowledge of California mental health regulations
Ability to build rapport quickly with diverse patient populations
Strong written and verbal communication skills
Comfort working in fast-paced hospital environments
