Roles & Responsibilities

Conduct comprehensive OASIS assessments for new patients
Manage Start of Care visits with compassion and expertise
Provide skilled physical therapy to patients recovering from falls and fractures
Travel across four beautiful rural counties serving Noble, Lagrange, Steuben, and DeKalb communities
Document visits accurately using tablet technology
Build meaningful connections with elderly patients in their homes

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong organizational and time management skills
