Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate social work services to patients in need
Conduct thorough psychosocial assessments and develop care plans
Coordinate care with interdisciplinary teams to ensure comprehensive support
Assist patients and families with resources and discharge planning
Complete 5150 evaluations and involuntary hold documentation when appropriate

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities required
Strong communication and interpersonal skills

Knowledge & Skills

Expertise in crisis intervention and mental health assessment
Knowledge of involuntary hold procedures and 72-hour evaluation protocols
Ability to work collaboratively within a multidisciplinary team
Understanding of community resources and patient advocacy
Professional documentation and care coordination skills
