Roles & Responsibilities

• Supervise and coordinate nursing care during the night shift to ensure resident safety and quality outcomes
• Conduct resident assessments, monitor health status, and communicate findings to attending physicians
• Document observations, assessments and progress notes accurately and timely in the medical record
• Update and implement care plans as resident needs and treatment plans change
• Serve as the on-duty manager for evenings and nights, handling staffing adjustments and call-offs
• Model best nursing practices, provide orientation to new staff, and mentor team members
• Act as liaison between residents, families and physicians, interpreting orders and educating as needed
• Respond quickly and decisively to emergencies and unexpected resident behaviors, implementing the emergency plan

Qualifications

• Current and valid Registered Nurse (RN) license for the state providing care (PA required)
• Experience with MatrixCare EMR required
• Two to four years of supervisory experience, preferably in geriatric long term care
• Strong written and verbal communication, coaching and conflict resolution skills
• Ability to stand and walk throughout the campus during the shift and assist with resident lifts
• Ability to lift up to 30 pounds for resident transfers and safe patient handling
• Compassion, empathy and sound clinical judgment in caring for older adults

• Effective physician communication and care coordination
• Familiarity with long term care regulatory expectations and resident rights
• Ability to lead teams, delegate appropriately and resolve staff issues respectfully
