Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes according to physician plans of care
Assess and evaluate patients' therapeutic needs, muscle strength, mobility, gait, and range of motion
Develop and update individualized rehabilitation plans while monitoring progress
Educate patients and families on therapeutic exercises, equipment use, and care techniques
Collaborate with physicians and care teams to report changes and coordinate treatment goals
Supervise home health aides and participate in case conferences for comprehensive patient care

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and rehabilitation techniques
Patient and family education on therapeutic appliances and exercises
Documentation and clinical reporting skills
Knowledge of Title 22, Medicare, Medi-Cal, and OSHA regulations
