Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day using the HCHB EMR system
Travel throughout the service area with mileage support provided (approximately 40 miles daily)

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong clinical skills for treating orthopedic and geriatric patients with multiple comorbidities

Knowledge & Skills

Physical Therapy expertise in home health settings
Proficiency with HomeCare HomeBase EMR system
Excellent communication and patient education skills
Ability to work independently while managing a daily caseload
Comfortable with tablet-based documentation and mobile care delivery
