Roles & Responsibilities

• Evaluate patients' functional status and create individualized treatment plans
• Implement therapeutic interventions to improve mobility, strength, and independence
• Monitor progress and modify treatment plans based on objective outcomes
• Collaborate with nursing and rehab team members to coordinate comprehensive care
• Document assessments, treatments, and patient progress accurately and timely
• Educate patients and families on safe mobility, home exercise programs, and fall prevention
• Train staff on proper transfer techniques and equipment use to promote safety
• Contribute to discharge planning and provide clear recommendations for ongoing care

Qualifications

• Active Physical Therapist license in Massachusetts or eligibility to obtain state licensure
• Graduation from an accredited physical therapy program (DPT preferred)
• Basic Life Support (BLS/CPR) certification
• Experience in skilled nursing or post-acute rehab settings is a strong plus
• Comfortable with electronic medical records and timely documentation
• Excellent communication skills and ability to work collaboratively with an interdisciplinary team

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy acceptable
• Graduation from an accredited physical therapy program

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and mobility assessment
• Knowledge of post-acute and skilled nursing rehabilitation practices
• Strong clinical reasoning and objective outcome measurement
• Proficiency with patient handling equipment and fall prevention strategies
• Ability to develop and teach home exercise programs and caregiver training
• Familiarity with documentation standards and regulatory compliance
• Good time management and caseload prioritization skills
• Collaborative approach to interdisciplinary care planning
