Roles & Responsibilities

• Assess patients’ physical function and mobility to develop personalized treatment plans
• Deliver therapeutic interventions including therapeutic exercise, gait training, and manual therapy
• Monitor patient progress and modify treatment plans to achieve measurable outcomes
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Document evaluations, treatment notes, and discharge plans accurately and timely
• Educate patients and families on home exercise programs and injury prevention strategies
• Ensure a safe environment by following infection control and safety protocols
• Participate in patient discharge planning and provide clear handoffs to next-level providers

Qualifications

• Current and unrestricted Physical Therapist license (ability to obtain California licensure if not already held)
• Basic Life Support (BLS/CPR) certification required
• Experience in inpatient rehabilitation, skilled nursing, or outpatient therapy preferred
• Comfortable providing care across weekends and collaborating in an interdisciplinary team
• Strong documentation skills and familiarity with electronic medical records
• Current immunizations and ability to pass facility background and health screenings

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual techniques
• Proficient in mobility training, transfers, and fall prevention strategies
• Knowledgeable about functional outcome measures and goal-oriented care planning
• Strong communication and patient education skills for diverse populations
• Ability to work independently while coordinating with interdisciplinary teams
• Familiarity with rehab modalities and adaptive equipment selection
