Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct OASIS assessments and start-of-care evaluations for new patients
Help elderly patients recover from falls and fractures with personalized care plans
Manage a manageable caseload of 4-6 patient visits daily using a point system
Document visits and track time efficiently with provided tablets
Build trust with patients and families in both home and community settings

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and start-of-care documentation
Comfortable working independently in rural communities
Strong communication and time-management skills
