Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehab potential
Create and update individualized plans of care in collaboration with physicians
Educate patients and families on therapeutic exercises and equipment use
Monitor progress, document outcomes, and communicate changes to the care team
Coordinate with home health aides and other professionals to support rehabilitation goals
Identify additional home health needs and connect patients with the right resources

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while thriving as part of a collaborative team
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy evaluation and treatment techniques
Home environment assessment and safety planning
Patient and caregiver education
Documentation and clinical reporting
Medicare, Medi-Cal, and Title 22 compliance
Team collaboration and case conferencing
