Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehabilitation potential
Create and update individualized treatment plans in collaboration with physicians
Educate patients and families on therapeutic exercises and equipment use
Monitor progress and communicate changes to the care team
Supervise home health aides and coordinate with other healthcare professionals
Document clinical notes and progress summaries to support quality care

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while thriving as part of a collaborative team
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and rehabilitation planning
Knowledge of Title 22, Medicare, and Medi-Cal regulations
Clear documentation and reporting abilities
Compassionate approach to patient and family education
