Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide home health physical therapy services to patients in a 60-mile radius around Valley City
Manage a caseload of 5-7 patients per day, focusing on post-surgical recovery, fall prevention, and hospice care
Utilize HomeCare/HomeBase EMR for documentation and care coordination
Develop and implement individualized treatment plans to restore function and improve quality of life
Collaborate with patients, families, and interdisciplinary team members to achieve optimal outcomes

Qualifications

Active ND Physical Therapy License required
Current BLS certification
Valid Driver's License and Auto Insurance
Minimum 1 year of PT experience, preferably in home health
Proficiency with HomeCare/HomeBase EMR preferred
No clinical interview required - quick offer process

Knowledge & Skills

Strong clinical skills in orthopedic and neurological rehabilitation
Excellent communication and patient education abilities
Ability to work independently and manage time efficiently
Familiarity with home health regulatory requirements
