Contract Type: Travel Contract

Shift Type: Rotating schedule including weekends and holidays, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Perform start of care, recertification, and evaluation visits for home health patients
Provide skilled physical therapy interventions to restore function and mobility
Educate patients and families on treatment plans and home exercise programs
Collaborate with clinical team members and document patient progress accurately

Qualifications

Valid Physical Therapist license in Kentucky
Current BLS certification required
Minimum 1 year of home health experience preferred
Strong clinical documentation and time management skills
Ability to work independently and manage a caseload effectively

Knowledge & Skills

Home health and Medicare guidelines knowledge
Excellent communication and interpersonal skills
Organizational and problem-solving abilities
Comfortable with travel to patient homes within assigned coverage area
