Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy to patients recovering from surgery, falls, and other conditions in their homes
Assess each patient's unique needs and create tailored treatment plans that promote independence
Collaborate with an interdisciplinary team to ensure seamless care across home health and hospice settings
Document progress accurately using HomeCare/HomeBase EMR
Travel within a 60-mile radius of Valley City to bring expert care directly to patients
Educate patients and families on exercises and safety strategies to support ongoing recovery

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of physical therapy experience, including previous home health work
Valid driver's license and auto insurance for travel within the service area
Comfortable working independently with minimal on-site supervision

Knowledge & Skills

Strong clinical skills in physical therapy for home health and hospice patients
Familiarity with HomeCare/HomeBase EMR documentation
Ability to manage a caseload of 5-7 patients per day
Excellent communication and patient education abilities
Reliable transportation and flexibility to cover a 60-mile radius
