Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Deliver exceptional physical therapy to patients in their homes, making a real difference in their daily lives
Conduct evaluations, OASIS assessments, and treatments to promote recovery and independence
Collaborate with healthcare teams to create personalized rehab plans
Travel to assigned territories like Napa, Vallejo, and Fairfield to serve diverse communities
Document visits thoroughly while building meaningful patient relationships

Qualifications

Active CA PT License
BLS certification (ACLS, PALS, NRP preferred)
Valid Driver's License with auto insurance
At least 1 year of experience
Willingness to see COVID+ patients and travel outside assigned areas as needed

Education

Physical Therapy degree from accredited program

Knowledge & Skills

Expertise in rehabilitation therapy and home health visits
Strong evaluation and OASIS documentation skills
Comfortable with productivity goals and per-visit workflows
Excellent patient communication and adaptability to varied territories
