Roles & Responsibilities

• Provide compassionate direct patient care, monitoring and documenting vital signs and changes in condition
• Administer medications and treatments safely and accurately per facility policy
• Collaborate with RNs and interdisciplinary team members to implement care plans
• Perform wound care and routine clinical procedures within LPN scope of practice
• Maintain clear, timely, and accurate patient documentation in the medical record
• Educate patients and families on basic care, meds, and safety in a friendly, professional manner
• Support safe patient transfers, mobility assistance, and infection control practices

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited practical nursing program (diploma/certificate)
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable with medication administration, basic wound care, and patient assessments
• Excellent communication skills, teamwork mindset, and commitment to patient-centered care
• Able to pass required background checks and drug screening

Education

• Diploma or certificate from an accredited practical nursing program
• Active LPN licensure for the state of Pennsylvania

Knowledge & Skills

• Strong clinical assessment skills and attention to detail
• Medication administration and safe documentation practices
• Familiarity with electronic health records and accurate charting
• Infection control principles and safe patient handling
• Time management, prioritization, and the ability to work in a fast-paced environment
• Compassionate bedside manner and effective communication with patients and families
