Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments according to physician orders and facility policy
• Assess, monitor and document patient status; report changes promptly to the care team
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Assist with activities of daily living and promote patient dignity and independence
• Maintain infection control, safety standards and accurate clinical records
• Support and mentor CNAs and other team members to maintain high-quality care
• Participate in care planning, family communication, and shift handoffs to ensure continuity

Qualifications

• Active LPN license in Pennsylvania (required)
• Current CPR/BLS certification (required or obtained upon hire per facility policy)
• Completion of an accredited Practical Nursing program or equivalent
• Valid immunizations and TB testing per facility requirements
• Strong communication skills and a patient-centered approach
• Ability to pass background check and drug screening as required by the facility

Education

• LPN diploma or completion of an accredited Practical Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Knowledge of fundamental nursing procedures and safe medication administration
• Proficient in patient assessment, documentation, and clinical observation
• Familiarity with electronic health records and standard charting practices
• Strong clinical judgment and ability to prioritize tasks in a fast-paced environment
• Teamwork, empathy, and effective communication with patients and families
