Roles & Responsibilities

• Provide compassionate, skilled nursing care to assigned residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document all care in the patient record
• Monitor patient status, recognize changes, and communicate effectively with the interdisciplinary team
• Assist with admissions, discharges, and transfers while ensuring continuity of care
• Support staff and help create a positive, resident-centered environment
• Follow facility policies, infection control standards, and safety protocols at all times

Qualifications

• Current LPN license in the state of Pennsylvania (or compact privileges where applicable)
• Active basic life support (BLS) certification or ability to obtain upon hire
• Comfortable working an evening schedule with weekend rotations and EOW requirements
• Clinical competence in medication administration, wound care basics, and resident assessment
• Strong communication skills and a collaborative attitude

Education

• Completion of an accredited LPN program (diploma or certificate) required
