Roles & Responsibilities

Provide compassionate direct patient care including medication administration, wound care, and routine assessments
Collaborate with RNs and the interdisciplinary team to implement patient care plans
Document patient information accurately in the EMR and communicate changes promptly
Monitor vital signs and respond to changes in condition with appropriate interventions
Educate patients and families on care plans, medications, and discharge instructions
Assist with admissions, discharges, and transfers to ensure smooth patient flow
Maintain compliance with facility policies, safety protocols, and infection control standards

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania or eligibility to obtain one
Basic Life Support (BLS) certification required
Clinical experience in long-term care, skilled nursing, or similar settings is a plus
Up-to-date immunizations and ability to pass required background checks
Strong communication, teamwork, and patient-centered care skills

Education

Graduated from an accredited practical nursing program (diploma or certificate)

Knowledge & Skills

Medication administration and safe documentation practices
Wound care and basic clinical assessment skills
Familiarity with electronic medical records (EMR) systems
Patient safety, infection control, and charting best practices
Ability to assist with activities of daily living and restorative nursing tasks
