Roles & Responsibilities
- Provide compassionate, skilled nursing care to long-term care patients in a home health setting as a member of the Per Diem pool
- Perform thorough patient assessments and develop/implement individualized care plans
- Administer medications and treatments safely while documenting care accurately
- Collaborate with interdisciplinary teams and communicate with patients and families to support continuity of care
- Complete Trinity specialty competency testing and other required competency evaluations
- Adhere to infection control procedures, participate in respirator evaluations and fit testing, and maintain required health screenings
Qualifications
- Active RN license (license/certificate verification required)
- BLS certification required
- Completion of Trinity specialty specific competency testing as part of onboarding
- Ability to complete required health screenings and immunizations: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, influenza vaccine
- Successful completion of Trinity 10-panel drug screen, respirator evaluation, fit test, color vision check, and physical/health statement
Knowledge & Skills
- Experience or familiarity with long term care and home health nursing practices
- Strong patient assessment and medication administration skills
- Accurate clinical documentation and comfort with electronic health records
- Infection prevention and control knowledge
- Good communication, teamwork, and time-management abilities
- Willingness to complete specialty competency testing and follow established protocols
