Roles & Responsibilities
- Provide high-quality skilled nursing care to homebound and hospice patients with compassion and clinical excellence.
- Assess patient condition regularly, develop and update individualized care plans, and document all findings accurately.
- Administer medications and treatments safely while monitoring response and reporting changes to the care team.
- Deliver comfort-focused, palliative and end-of-life care in collaboration with families and interdisciplinary partners.
- Educate patients and caregivers about care plans, symptom management, and available community resources.
- Coordinate with physicians, therapists, social workers, and community agencies to ensure seamless care transitions.
- Maintain infection control standards, complete required screenings, and uphold organizational policies and procedures.
- Participate in competency testing and continuing education to maintain clinical skills and certifications.
Qualifications
- Active RN license (must be verified) — meet state requirements for practice
- Current BLS certification required
- Experience or strong interest in long-term care, home health, and hospice settings preferred
- Willingness to complete Trinity Specialty Specific Competency Testing
- Ability to meet health screening requirements (fit test, 2-step PPD or IGRA, drug screen, immunizations)
- Strong clinical assessment, documentation, and patient/family communication skills
Knowledge & Skills
- Skilled in patient assessment, medication administration, and symptom management
- Comfortable providing palliative and end-of-life care with empathy and clinical expertise
- Proficient documentation and familiarity with electronic medical records (EMR) workflows
- Excellent communication and care-coordination abilities with interdisciplinary teams
- Infection control practices and adherence to clinical safety protocols
- Time management, prioritization, and adaptability in home care environments
- Willingness to complete specialty competency testing (Trinity) and required health screenings
