Roles & Responsibilities
- Assess residents’ clinical status and changes in condition with compassion and precision
- Administer medications and treatments safely and in accordance with facility protocols
- Monitor vital signs and document observations thoroughly in the health record
- Collaborate with interdisciplinary team members to create and update care plans
- Provide resident and family education to support health, safety, and independence
- Advocate for residents’ needs and ensure dignity in all aspects of care
- Perform competency-based tasks following Trinity specialty testing and skills checklist
- Maintain accurate and timely documentation for licensing and quality standards
Qualifications
- Current, active RN license appropriate for Pennsylvania practice
- BLS certification required
- ACLS certification required
- Experience or interest in long-term care (LTC) environments
- Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Ability to pass License/Certificate Verification and required health screenings
- Successful completion of background and 10-panel drug screen as required
Knowledge & Skills
- Strong clinical assessment and critical thinking skills in long-term care settings
- Medication administration and IV/infusion familiarity as applicable
- Knowledge of infection control, immunization requirements, and TB/PPD testing protocols
- Comfortable with electronic health records and accurate documentation
- Excellent communication and teamwork skills with residents, families, and staff
- Ability to follow facility protocols, checklists, and competency testing processes
