Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing and long term care environment
• Assess residents regularly, document findings, and implement individualized care plans
• Administer medications and treatments safely and accurately according to policy
• Monitor changes in condition and escalate concerns to interdisciplinary team members
• Educate residents and families about care needs, safety and discharge planning
• Collaborate with nursing peers, facility staff, and leadership to ensure continuity of care
• Maintain accurate, timely documentation in the electronic medical record
• Adhere to infection control practices, safety protocols, and facility policies

Qualifications

• Registered Nurse (RN); active license required and License/Certificate Verification will be completed
• BLS (Basic Life Support) certification required
• Completion of Trinity Specialty Specific Competency Testing as part of onboarding
• Current fit test and respirator evaluation required prior to assignment
• Up-to-date immunizations and health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
• Negative Trinity 10 Panel Drug Screen and completed physical/health statement
• Experience in Skilled Nursing Facility (SNF) or Long Term Care (LTC) preferred but motivated RNs are encouraged to apply

Knowledge & Skills

• Strong clinical assessment and critical thinking in the LTC/SNF setting
• Medication administration and documentation accuracy
• Familiarity with long term care workflows and regulatory expectations
• Effective communication with residents, families, and interdisciplinary teams
• Ability to perform and document clinical procedures and nursing tasks
• Infection prevention and control practices
• Comfort learning and demonstrating competency through specialty testing
• Adaptability to varied shift patterns and resident care needs
