Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing and long term care environment
- Assess residents regularly, document findings, and implement individualized care plans
- Administer medications and treatments safely and accurately according to policy
- Monitor changes in condition and escalate concerns to interdisciplinary team members
- Educate residents and families about care needs, safety and discharge planning
- Collaborate with nursing peers, facility staff, and leadership to ensure continuity of care
- Maintain accurate, timely documentation in the electronic medical record
- Adhere to infection control practices, safety protocols, and facility policies
Qualifications
- Registered Nurse (RN); active license required and License/Certificate Verification will be completed
- BLS (Basic Life Support) certification required
- Completion of Trinity Specialty Specific Competency Testing as part of onboarding
- Current fit test and respirator evaluation required prior to assignment
- Up-to-date immunizations and health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
- Negative Trinity 10 Panel Drug Screen and completed physical/health statement
- Experience in Skilled Nursing Facility (SNF) or Long Term Care (LTC) preferred but motivated RNs are encouraged to apply
Knowledge & Skills
- Strong clinical assessment and critical thinking in the LTC/SNF setting
- Medication administration and documentation accuracy
- Familiarity with long term care workflows and regulatory expectations
- Effective communication with residents, families, and interdisciplinary teams
- Ability to perform and document clinical procedures and nursing tasks
- Infection prevention and control practices
- Comfort learning and demonstrating competency through specialty testing
- Adaptability to varied shift patterns and resident care needs
