Roles & Responsibilities

• Provide compassionate direct patient care, monitoring and supporting daily living needs
• Administer medications and treatments safely and document accurately
• Perform focused patient assessments and report changes to the care team
• Maintain clear, timely documentation in the patient record
• Collaborate with nurses, therapists, and physicians to implement care plans
• Educate patients and families about care plans, medications, and follow-up
• Support infection control practices and ensure a safe environment

Qualifications

• Active LPN license in New Mexico (or eligible to obtain) — required
• Completion of an accredited practical nursing (LPN/LVN) program
• Basic Life Support (BLS) certification preferred or willing to obtain
• Current immunizations and ability to pass background and drug screening
• Strong communication skills and a patient-centered attitude
• Previous experience in a long-term care or skilled nursing setting preferred

Education

• Vocational Nursing diploma or certificate from an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment and vital sign monitoring
• Familiarity with electronic medical records and charting
• Wound care basics and comfort with common nursing procedures
• Infection control and safety protocols
• Time management, prioritization, and teamwork in a clinical setting
• Compassionate patient and family communication
