Roles & Responsibilities

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders using evidence-based practice
• Develop and implement individualized plans of care and adjust treatments based on patient progress and standardized assessments
• Document all clinical care and outcomes accurately in the electronic health record and prepare comprehensive discharge summaries
• Provide counseling and education to patients, families, caregivers, and other care team members to support optimal outcomes
• Communicate patient progress, barriers, and treatment plans clearly with supervisors and interdisciplinary team members
• Accurately record treatment minutes and maintain regulatory-compliant patient records
• Participate in facility and company-required trainings, patient care conferences, utilization reviews, and rehabilitation meetings
• Obtain necessary DME/supplies per clinical requirements and support care coordination
• Supervise and support CFY and student Speech-Language Pathologists to foster the next generation of clinicians

Qualifications

• Active ASHA CCC (Certificate of Clinical Competence) required
• Active Missouri State Speech-Language Pathology license required (Compact allowed)
• Minimum 1 year of SLP experience required; new grads OK if they have Level II experience (not just Level I observation)
• LTC or Rehab experience required
• Able to supervise and support CFY or student SLPs
• 3 years experience holding active CCS preferred
• Ability to provide one professional reference at time of submission

• Evaluation and treatment planning for communication, cognitive, and swallowing disorders
• Use of standardized assessments and evidence-based clinical interventions
• Electronic health record (EHR) documentation and discharge summary preparation
• Clinical outcome evaluation and modification of services based on progress
• Patient, family, and caregiver counseling and education
• Regulatory compliance (ASHA and state practice acts) and documentation requirements
• Group therapy implementation and productivity management
• Understanding of DME procurement, Medicare A/B, and managed care processes
