Roles & Responsibilities

• Provide skilled physical therapy evaluations, treatments and discharge planning for home health patients across assigned East Bay territories
• Complete OASIS assessments, Routine visits, Evaluations, Re-Evals and Discharges accurately and on time
• Document clinical findings and visit details thoroughly to support patient care and per-visit approvals
• Collaborate with nurses, physicians and the care team to develop individualized care plans that improve patient function and safety at home
• Maintain productivity expectations by managing your weekly caseload and travel to meet visit targets when required
• Adhere to agency and client protocols including infection control and safe patient handling, including willingness to treat COVID+ or PUI patients when assigned

Qualifications

• Current California Physical Therapy License (required)
• BLS certification
• Valid Driver's License, current auto insurance and vehicle registration for travel between patient homes
• Current TB test (annual), MMR, Varicella, HepB, TDap (10 years), annual Flu vaccination or exemption documentation
• Negative drug screen and up-to-date physical exam per agency/client requirements
• Home health experience with OASIS assessments and documentation strongly preferred
• Comfortable with per-visit billing models and meeting productivity expectations
• Willingness to travel within assigned territories and provide weekend coverage when needed

Knowledge & Skills

• Proficient in home health patient evaluation, mobility and safety assessments
• Strong OASIS knowledge including SoC, ReCert, RoC and Discharge processes
• Accurate and timely documentation skills to support visit-based hour approvals
• Excellent patient and family education and communication skills
• Ability to manage time effectively while traveling between patient homes
• Familiarity with infection prevention, PPE and care of COVID+ or PUI patients
