Roles & Responsibilities

• Assess patients and families to identify psychosocial needs and develop individualized care plans
• Provide counseling, crisis intervention, and emotional support in an acute care setting
• Coordinate discharge planning and community resources to ensure safe transitions of care
• Collaborate with physicians, nurses, and multidisciplinary teams to advocate for patient-centered care
• Document assessments, interventions, and progress accurately in Cerner
• Act as a patient and family advocate to address barriers to care and access services

Qualifications

• Valid professional social work license required (License to submit: Yes)
• BLS certification required
• Minimum 2 years of experience (acute care experience preferred)
• Familiarity with Cerner or similar electronic health record systems
• Willingness to complete required orientation (12 hours) and follow hospital policies

Knowledge & Skills

• Clinical Social Work in acute care environments
• Discharge planning and community resource coordination
• Electronic charting/documentation (Cerner)
• Basic Life Support (BLS) competency
• Culturally sensitive communication and crisis intervention skills
• Interdisciplinary collaboration and care advocacy
