Roles & Responsibilities

• Provide comprehensive psychosocial assessments and develop patient-centered care plans
• Conduct CA 5150 evaluations and follow state-required procedures with accuracy
• Coordinate care and discharge planning across multidisciplinary teams to ensure smooth transitions
• Document assessments, plans and progress thoroughly in Cerner
• Serve as a clinical resource to patients, families, and staff, advocating for patient needs
• Collaborate with inpatient teams in an acute care setting to support quality outcomes
• Float to related units as assigned while maintaining continuity of care

Qualifications

• LCSW licensure (required)
• BLS certification (required)
• 5150 certification in California (required) — completion of training program and passing the exam (80% required)
• Prior CA Dignity (CommonSpirit/CHI/Dignity) experience required (travel or permanent) — candidates without this will not be considered
• Minimum 2 years of relevant social work experience (acute care preferred)
• Two supervisory references at submittal (charge experience accepted): one within 12 months and one within 3 years
• Candidates must not have been directly employed by any CommonSpirit-associated facility within the past year; PRN employment through outside firms is acceptable

Knowledge & Skills

• Strong care coordination and discharge planning skills
• Experience working in acute care environments
• Proficient charting and documentation in Cerner
• Clinical skills in behavioral health and crisis evaluation (5150 process)
• Ability to collaborate with interdisciplinary teams and communicate clearly with patients and families
• Adaptability to float to other units and manage varying workloads
