Roles & Responsibilities

Assess residents using the full nursing process to identify needs and changes in condition
Develop, implement and update individualized care plans in collaboration with the care team
Administer medications and treatments safely and document all care thoroughly
Interpret diagnostic tests and integrate findings into resident care decisions
Provide resident and family education to support optimal outcomes and independence
Collaborate with interdisciplinary team members to coordinate care and achieve quality outcomes
Contribute to performance improvement activities that enhance resident care and safety

Qualifications

Current California RN license (required)
Current CPR certification (required)
Minimum of 1 year of experience as a Registered Nurse (required)
Graduate of an accredited school of professional nursing

Education

Graduate of an accredited school of professional nursing

Knowledge & Skills

Skilled in the nursing process: assessment, diagnosis, care planning, implementation and evaluation
Medication administration and safe documentation practices
Ability to interpret diagnostic tests and incorporate results into care plans
Experience in Skilled Nursing Facility (SNF) and geriatric care
Strong collaboration and communication with interdisciplinary teams
Resident education and performance improvement mindset

Benefits

Sign-on bonus: $7,500
Compensation structure with 30 pay increments to reward up to 30 years of relevant experience
