Roles & Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care using evidence-based practices
• Develop and implement individualized treatment plans and adjust interventions based on patient progress
• Administer manual techniques, therapeutic exercises, modalities, ambulation training, and manage assistive/supportive devices
• Perform standardized assessments and document outcomes to guide clinical decisions
• Provide patient, family, and caregiver education and counseling to support recovery and independence
• Communicate effectively with supervisors and interdisciplinary team members about patient progress, barriers, and plans
• Prepare comprehensive discharge summaries and maintain accurate clinical records in the electronic documentation system
• Accurately record treatment minutes and comply with regulatory and clinical documentation requirements
• Participate in patient care conferences, utilization reviews, and other required meetings and trainings
• Obtain necessary DME or supplies and coordinate resources to support patient care
• Supervise and mentor physical therapist assistants, rehabilitation aides, and students while adhering to professional practice acts

Qualifications

• Missouri State License — required (Compact allowed)
• Minimum 1 year of Physical Therapy experience — required (LTC or Rehab experience required)
• New graduates considered if they have Level II experience (not just Level I observation)
• Familiarity with APTA and State practice acts, professional conduct, and ethics
• Ability to document thoroughly in electronic medical records and accurately record treatment minutes
• Experience supervising PT assistants or students is a plus

Knowledge & Skills

• Patient assessment, treatment planning, and rehabilitation expertise
• Wound care knowledge and skillful application of wound-related interventions
• Proficiency with standardized assessments and evidence-based practice
• Manual techniques, therapeutic exercises, and use of modalities
• Experience with assistive devices and durable medical equipment (DME)
• Strong documentation skills and comfort with electronic documentation systems
• Ability to communicate effectively with patients, families, and interdisciplinary teams
• Skill in preparing discharge summaries and tracking treatment outcomes
