Roles & Responsibilities

• Provide patient-centered physical therapy during home visits, focusing on functional mobility and safety
• Assess patient needs, create and implement individualized care plans, and set measurable goals
• Document all visits accurately and timely using HCHB to ensure continuity of care
• Manage an efficient caseload (typically 5–6 patients per day) while prioritizing quality outcomes
• Educate patients and caregivers on home exercise programs, safe transfers, and fall prevention
• Collaborate with nurses, social workers, and other team members to coordinate holistic care

Qualifications

• Minimum 2 years of home health physical therapy experience (explicitly required)
• Home health experience for therapy positions is a must; hospice experience is noted for nursing roles
• Experience using HCHB (Homecare Homebase) for documentation and scheduling
• Reliable vehicle for patient visits and valid driver’s credentials
• Ability to work Monday–Friday daytime schedule; strong communication and organizational skills
• Active Physical Therapist license or ability to obtain NC licensure (state requirements apply)

Knowledge & Skills

• Skilled in home visit assessments and tailoring interventions to the home environment
• Proficient with HCHB documentation workflows and electronic charting
• Strong patient education and caregiver training capabilities
• Comfortable managing 5–6 patient visits per day with efficient time management
• Excellent clinical judgment for discharge planning, mobility, and safety recommendations
• Team-oriented communication with interdisciplinary home health staff
