Roles & Responsibilities

• Assess patient function and needs to create individualized treatment plans
• Develop and document clear, goal-driven plans of care in Epic
• Deliver skilled therapeutic interventions including gait training and mobility work
• Perform transfers and lift training safely using Hoyer and sit-to-stand devices
• Monitor and report vital signs and patient response to treatment
• Collaborate with interdisciplinary team members to support discharge planning
• Educate patients and families on home exercise programs and safety strategies
• Reassess progress, update short-term and long-term goals, and communicate changes

Qualifications

• Active state Physical Therapist license required (pending licenses not accepted)
• Minimum 1 year acute care hospital experience preferred; acute rehab experience or willingness to float with orientation
• Charting experience required; Epic experience preferred
• BLS certification preferred
• Comfort working with orthopedics, cardiopulmonary, neurology, and geriatric populations
• Ability to float to acute rehab unit as needed and participate in on-call coverage
• Locals accepted; RTO (requested time off) must be included at time of submission

• Vital sign monitoring and cardiopulmonary knowledge
• Orthopedics, neurology, and rehab-focused interventions
• Time management and prioritization in a busy acute care setting
• Strong communication and patient/family education skills
• Familiarity with Trauma Level II environments and interdisciplinary teamwork
